How do I submit a rating on my provider's profile?

To submit a rating to your provider's profile, you will need to find his/her profile first. Once you are on the provider's profile, the Rating Form can be found at two locations as shown below.

Interacting with the Rating Form will trigger a pop-up where you can input the details of your rating.
